WCS Temporary D-35 Request Process

Due to the ongoing network issues and inaccessibility to the CARDS web portal, please follow the instructions below to submit a D-35 form to request a Permanent Partial Disability (PPD) evaluation or an Independent Medical Examination (IME) for the purpose of determining if there is a ratable impairment or a dispute to a previous rating.

Email your completed D-35 request form to medunit@dir.nv.gov for processing.

Once completed the Med Unit will email the submitter and the assigned rater, a copy of the completed D-35 will be attached.

This process has been developed to provide a bridge for D-35 processing during the ongoing network issues. You will be notified when CARDS functionality is restored and the return to normal processing of D-35 requests and Claims Indexing.
